An antibiotic is usually a 5 or 7 day course, if after that course you still feel as if you cannot hear properly, I would firstly have your hearing tested by an audiologist, and thereafter she / he will guide you as to whether you need to see the ENT again for further management. It may be that the antibiotics are not as effective in reducing the fluid build up, which in some cases requires draining of the ear in order to start hearing better again.
