jaw pain

Good day.

ten years ago when i was in matric i had surgery to correct my overbite. then a few years after that i had a root canal in my left lower molar (the 2nd one from the back) Then 5 years ago i broke that same tooth and it crowned. since the middle of June I have had severe pain based in and around that area. I went to my dentist (who has been my dentist for over 15 years and who I trust with my life) and he prescribed antiobiotics (2 courses) as the gums were sore and my the muscles around the joint were in sever spasm. Both courses of antibiotocs made a huge difference. but its still not 100% and my dentist is concerned as i still flinch when he taps that tooth and progressivly it has worsened again. He has spoken to the maxillo facial surgeon who operated on me in 1998 and we are waiting for a response from him. My dentist said that it could be one of three things, 1 the tooth might have a lateral crack which he cant see. 2 there might an infection around the outside of the root 3. the one titanium screw (which is right underneath my root) might be infected. it has been more than two weeks and i still haven now word from the surgeon. what is my next step and what is his please? will surgery be nessecary? anything else i should know? Without the benefit of an x-ray it is very difficult to say. The best option may be for you to seek an opinion from a second maxillofacial surgeon. Tell your dentist to refer you to someone else

thanks for your speedy reply.

the dentist did mention quite early on when the pain started that the surgeon might have to operate and go in from just below my teeth and see what the problem is and clean it up. I know the maxillo facial guy is SUPER busy and he is an awesome doctor and very very anal when it comes to deciding what to do and its a great characteristic to have in someone in his profession but i' m getting frustrated with the pain. also what i forgot to metion is that the dentist cleaned out the roots and put some meds in which did help a bit. but then not for long. i wont hold you too it but what is the worst case scenario according to you? i sometimes feel like ripping my jaw out! thanks again!

ps the dentist was concerned about a small black dot above the one screw on one of my smaller xrays. that specific one was taken two weeks ago wednesday. that was the last one.

i' m not too keen to change surgeons as he has all my records but if push comes to shove i def will do that!
