persistent chronic cough for 20+ years

For over 20 years I have suffered with a cough that disrupts everyone around me. I have usually between 8 - 10 episodes per day consisting of 20 - 30 minutes of constant coughing. The first 5 - 10 minutes are a dry cough which is most uncomfortable & I must cough very hard to loosen what feels like fluid in my lungs. Once it has loosened my cough sounds gurgly and it's a relief because it's productive, although it's unpleasant for others to hear. I was just recently dismissed from the cough clinic at the hospital after 3 years going through all the tests and prescriptions they could think of with no answers. I have been told that my lungs are 30% larger than average, but this doesn't affect my lung health in any way. Looking for any new ideas of what I could possibly try.

I would suggest you have a trial of an inhaled nasal spray and antihistamines, and/or alternatively strong antacid therapy. These are 2 common causes of occult chronic cough. I would also recommend that you see a pulmonologist.
