URGENT! sick toddler, again

It started last thursday with a fever and he vomited once. Friday he started with runny poos and still fever. He is pale and has dark rings under his eyes. The week-end I had him on lactose free NAN, juice and fruits, and that's all he wanted to eat. I gave cowsmilk once and then he vomited the entire bottle up. I noticed that every time he tries to eat, because he wants to, he pushes his tummy in and out and then refuses to eat. But he has been drinking plenty of fluids. I managed to get some cracker bread into him on the week-end but that's all. His ears are also leaking fluid, had grommets put in in May this year. He has been sleeping very bad since last THursday. This morning his teacher at creche confirmed that he looks thinner and pale. Should I be taking him to the Ped. at this stage or should I ride it out a few days more to see improvement as he is keeping fluid down. His poos are soft but not like diarhea. What could be wrong with my boy, 19months?

I think it`s teeth,my boy is 15 months and often has a fever for 2 to 3 days, a got some Ponstan for him at the docter and it helps for the fever and "setpille" if it gets more bad.He also drank nan Lactose free up until 1 year,I tried to take him of it about 10 months, but he trows up everything and then started fever and runny nose,so he had a bad cold and at 1 year i had to make a choice between Nido and 2%cowsmilk(the box)And wonderfull for both of us he did not have a bad reaction to it.The only thing now is teeth!!He had a swelling under his left ear this weekend with fever ,i thought it was "pampoentjies",but the next evening it was gone,so it must have been teeth!!
But take him to the doctor as they say,its the best!
