Nappy rash

My 3 week old baby, has a very bad nappy rash and really dont know what to do. It looks very sore.
I have been using antipoel ointment and stopped using baby wipes, i now use cotton wool and warm water. That seemed to have helped for her previous rash, but now she has a very bad rash in one spot and is very raw, the ointment isnt sticking to the place where the skin is so raw. Please could you advise me on what i can do.

Many thanks

It is unusual for a young babay to have such a severe nappy rash that does not respond to the usual nappy creams. Try changing the brand of nappies you are using as a start and apply Bepanthen nappy cream to your baby's nappy rash. If this does not clear you should consult your paediatrician It would be a good idea to check your baby's zinc level in her blood.
