This is a very good question and your description of your son’s stomach pain is excellent. It is unlikely, but not impossible that he has appendicitis at such a young age. With his high fever and lower abdominal pain a urinary infection must be excluded. Another possibility is an illness called mesenteric adenitis where the lymph nodes in the abdomen become temporarily enlarged as a result of infection causing quite significant abdominal pain. It is essential that your son is seen at your nearest paediatric emergency unit.
