Extreme weight loss due to diabaetes

Hello sir/mam, I am ashish and i want to ask some questions about my father, he is 58 years old and he has a diabetes. He also has diabitic retinopathy and he lost 95% of his vision in the right eye. He is taking Amaryl m1 g tablet daily. He also extremely lost his weight and got so much physically weak because of diabetes and he also has so much pain in left shoulder. My question is i am unbale to find a doctor for dietry & neutrition and I don't know whether he has a type 1 diabetes or type 2. Please help me to suggest some tests or some neutrition and dietry advice or guide me for which specialist/expert should I visit for my father's physical weakness issue. Thank You.

Your father has complex medical issues, and needs to be assessed by an Endocrinologist or a specialist Physician. He may also  need referral to a Neurologist if he has complications from diabetes affecting his nerves
