Q.

Bad bouts of severe itching skin

Hello. I have had bouts of severe itching skin for a few years now. Come on unexpectantly. Usually starts around the mouth and progresses to the scalp. Lower arms, lower legs, the genital area under breasts. Usually essentially the whole body. If I scratch then leaves red lines. On testing, I know my IgE levels were high at one stage. I have tried seeing if it is medication and food. Still found no answer. Now in the last 2 weeks on 3 occasions I wake up with a very flushed face that burns. Mostly on cheeks and chin. I take Dalit daily. When these attacks occur nothing seems to relieve them. Can literally take a combination of Phenergan 25mg about 12 a day including loratadine, allergen, etc at random just to find relief. In last week I had 2 cortisone injections which didn’t seem to help at all. In fact, it seemed to get worse. I have had asthma as a child but am under control on Symbicort. I have a reflux problem and recently started omeprazole again which does help. Also severe pain in the maxillary area. Seems to come from a central point. Extensive tests and doctors cannot find out what is wrong. Sinuses and Mri are 100%. It is almost a constant throbbing pain. I’ve had 3 teeth even removed. Pain meds I take do not help. Can the itching be something like mastocytosis? It seems that that should be relieved by antihistamines though. I am desperate for help. I have seen Dr Morris many years ago and I did skin tests on me which produced reactions but just the usuals. He gave me Atarax. It helped them. But it is no longer available. I also find it hard to sleep. Use sleeping tablets but they don’t always help. Years ago I was diagnosed with fibromyalgia. I stopped that medication due to itching as well as wait gain. Started tegretol and arcoxia and trepiline recently to relieve pain. I know I’m allergic to codeine products and bactrim. I stopped all medication a year ago due to the desperation of itch but didn’t resolve problem. So I started excluding foods eg gluten and dairy. Then tried eliminating night shade veggies all no difference. So I’m really confused and desperate. Will I be able to be helped Thank you very muvh

You certainly seem to be struggling with your allergies. Your condition sounds very complex and perhaps various different factors are implicated. It may be worth reviewing your allergies again. Perhaps your doctor could arrange a comprehensive allergy blood test called at ISAC component allergy test or the new ALEX/MADx test available at the UCT Allergology Unit in Cape Town. CAST testing can also be done for hypersensitivity to the common preservatives and food colorants, as well as checking your Tryptase levels to exclude Systemic Mastcytosis.
