Hi Jane, thank you for the question.

The area that you describe is known as the Melomental Fold and the Pre-Sulcas and the best approach is with combination treatments addressing both these areas. However, this is a difficult question to answer over the net and one should examine the area of concern first and then decide on the appropriate treatment regime as the depth of the Melomental fold/groove and the pre-sulcas depression and the degree of improvement you would like to achieve will affect the treatment choice.

Treatment Options:

Surgery: Face Lift or SMAS

Combination treatments (non-surgical):

Fillers: fillers have an amazing ability to remove the depression noticed in both the Melomental fold and the pre-sulcas area if applied correctly.

Skin tightening and lifting technologies:
You have three options to choose from namely Syneron’s Refirme, Titan and Thermage. One can expect a maximum improvement of around 30% with these technologies but the following should be kept in mind:
The side effect profiles of these technologies vary.
The pain experienced with the treatments varies.
The length of time for each of these treatments varies.
Results are seen over a 6 month period.

Botox: Botox can be injected into the muscles responsible for pulling the lower half of the face down thereby creating a mild lift to improve the overall effect of the above treatment options.

Skin Rejuvenation:
By adding skin rejuvenation to the above regime improved results can be expected and various options exist such as Fraxel, IPL and Bi-Polar Radiofrequency (Syneron eMax and eLight) and IPL alone (Ellipse).

In my experience a number of patients are happy with the end result when only fillers and Botox are chosen, while others are happy but would like further improvements and decide on using skin tightening technologies as well.
