The swing towards natural cycle IVF is increasing significantly, particularly as laboratory techniques improve. Although the pregnancy rates do not at present equal that of stimulated IVF cycles, success is reasonably good enough to warrant recruitment of patients into a natural cycle program. At present a success pregnancy rate in order of 20-30% can be expected depending on age and other factors (serious sperm abnormalities etcetera). Therefore in cases where a simple tubal obstruction or minor endometriosis or moderate sperm abnormalities are present this protocol has reasonable value. However, one should be willing to accept abandonment of cycles or in some cases there may be no embryo to transfer if fertilisation in the laboratory has not occurred. On the cost benefit scenario one could expect a cost of approximately 20% of the normal IVF rate.
