Essentially, smoking causes deterioration of the lining of the blood vessels all over the body resulting in an increased risk of heart attacks, heart failure, strokes, kidney failure, etc.  For the same reasons smoking causes reduced blood supply to the hands and the legs and may results in pain during activity.  It is directly toxic to the lungs resulting in damage to the lungs (COPD), and lung cancer in susceptible individuals.  There is a lot of online literature about the dangers of smoking.
