Endless cough

Dear Professor. Dherda I am Asthmatic since I was 18 years old. On Seratide, Asthavent as needed. I take 1 Austell 10mg per day. I was given Flomist/Flixonase but within 45 minutes of taking it I have such a bad headache I cannot function - so I stopped. I have had pneumonia 3 x in the last 5 years. I am 54, female. I try to look after myself. I have seen pulmonologists in the past who said my lung function is excellent, x-rays normal for me in that they show asthma (spidery white lines across). I've had a cough for a year. Sometimes it sounds like bad bronchitis, sometimes mild (phlegmy) intermittently in the day say like 4-5 barking episodes. They've done 3 x-rays all clear, ECG good, blood tests - kidneys, liver, spleen, gall bladder all normal. Did sonar - lady parts post menopause, did colonoscopy all good. My family has reflux and I take 1 x Lansoloc a day. I used to take Nexium when I was on med aid. Dr. told me they are the same thing and just as strong, but are they? I think it may be reflux. Of course the cough is worse during allergy season. 1)Pls let me know about the reflex meds. 2)Should I have a gastroscopy? 3)I have IBS 4)Milk affects me so I only have it in tea, does bread cause a cough in asthmatics? 4)I do sinus rinsing at night with pink salt & little baking powder - do you think it actually helps or not? I go to the shops and hack and everyone looks at me like I am contagious. My cough is embarassing me. I feel fine. I just do suffer from sinus pain a lot. Always have. Get lots of sinus headaches since teens. Have seen ent said I'm allergic. I also feel as if something may be wrong but nobody is able to pinpoint it. I actually don't know what Dr. or what test to have. I am at the state, having said this, I have had excellent treatment and see wonderful Doctors even if you have to wait, which I don't mind as I am trying to get a biz off the ground after being retrenched so for good and free care I will wait and wait and wait. It's worth it.

It sounds very much like you have chronic hay fever with post-nasal drip.  This is the commonest cause of persistent cough in those with allergies. I know you stopped the Flomist.  I would suggest you talk to your GP and ask him to consider prescribing Mometasone nasal spray 1 puff/daily in each nostril.  You will have to find a nasal steroid that you can tolerate.  The next question is why are you not tolerating nasal steroids?  This is unusual.  Did an ENT have a look to see whether you have polyps?  You also need to try 3 or 4 different types of antihistamines to see what works for you to dry out your nasal passages.
