Viral Dysentery and very unhappy 3 year old....

Hi Dr
Apologies in advance for the long post...

My little guy was diagnosed with viral dysentery on Saturday morning after having a really bad Friday night that saw him gagging and vomiting (even though he has had an antireflux op) and we took him to the ER, the paed wasn’ t sure what was wrong and suggested a suppository even though his stomach was soft. Saturday morning Tay had a huge big blood and mucus pooh after which l called the paed on duty and he was admitted. He was running a temp of 38.3’ C, slight red throat, gagging, complaining of sore stomach,, trying to vomit and would vomit even when l tried to feed him through the peg. He was given 7.5ml calpol for pain, zofron through drip for nausea, and something with a N for the acid in his stomach while being rehydrated @ 60ml per hour.

Blood showed it was viral and the diagnosis was viral dysentery. He still wouldn’ t eat or drink at all.

He looked ok yest morning and after we managed to keep 360ml down through the tube, we went home late last night. Then all hell broke loose. He screamed solidly for over an hour, resisted any attempts for me to comfort him etc and kept telling me his mouth is sore. The paed told me at 3.5yr he is actually referring to his throat and not the mouth that is sore but he won’ t even take his dummy and it looks as if he has sores in his mouth now. I called the paed last pm and was told to change his meds to 5ml stopayne every 4-6 hours and 10ml Ponstel every 6-8 hours and that dysentery does cause sores in the mouth? Also to give him andolex ointment (which he promptly vomited out all over me last night after constant gagging for about 10 min). This is A LOT of pain meds but it is not touching his pain and he is crying in his sleep and miserable. Is there another combination I can try?

I have seen Tayden sick many times but I have NEVER seen him in pain like this. Is it normal?

Dr, l leave for business on Thursday for 12 days and I am going to be out of the country. I cannot get out of it. I am a wreck leaving him like this –  how long will this virus last and what else can l do for pain and try and get him over this?

It is difficult to be sure about Tayden's illness without being able to see him,but he seems to be developing a condition called herpes stomatitis [mouth ulcers] in addition to his viral gastro-enteritis.His entire illness may be due to a severe herpes virus infection. He should be started on an antiviral medicine immediately such as Acyclovir 200mg 5 times daily for 5 days.He will need to stay in hospital to ensure that his hydration is adequate. It is going to be very hard for you to have to leave him at this stage.Let me know about his progress.
