very pale 4 yo

Dear Prof

My 4 yo son (the one with Arnold Chiari 1) is very pale. He was tested for anaemia in Des, but it came back negative. The only other symptom is a persistent swollen lymph gland in his left neck/jaw area, which is small, soft and movable. He has also been hoarse for 3.5 weeks now. What else could it be?

You have excluded anaemia as a possible cause for your son being so pale. Other possible causes need to be considered now. The gland you mention is known as the tonsillar lymph node because it drains the tonsil area. It is possible that he has enlarged and infected tonsils. Even more likely is that he has an infection of his trachea known as tracheitis as this usually lasts a long time. It would be best to ask your paediatrician or an ENT specialist to see your son as it is important to find out why he has such prolonged hoarseness.
