Premmie twins - concerns

Hi there Doctor
I have a friend with twins, now 16 months old (birth age). They were born at 28 weeks. They only weigh about 7.5kg each, and they are very petite (small heads as well) although their length is correct for their age. They get ill very easily and also get diarrhoea quite often. They are terrible sleepers, waking up 5-6 times per night and sometimes screaming for long periods. Their ped as well as a dietician says all of this is quite fine - they haven'  t done any tests for allergies or milk intolerance. The dietician advised the parents to mix their Nan with full cream cow'  s milk to help them pick up weight. My friend is applying strict sleep training and sometimes it works  sometimes not at all.
My friend is not 100% satisfied with their expanations. I also feel she should get a 2nd opinion and even take them for allergy and other tests. What do you suggest?

Paediatrician

Both of these children are well below their expected weight even allowing for their prematurity.They should weigh at least 10 kg now. They could well have an intolerance or allergy to milk or other foods.I am concerned about their small heads. They definitely require 2 asessments. Firstly they need to be seen by a developmental paediatrician and then by a paediatric allergy expert.There are experts at most of the paediatric departments at the main teaching hospitals.
