Pneumonia treatment

Dear Prof

My 2.5 yo son was diagnosed with bacterial pneumonia yesterday afternoon. He had a dry cough with moderate fever starting Monday aftternoon, and fever of over 40 yesterday afternoon with crackly breathing, so very sudden. Our paed prescribed Zinnat for 7 days, and said nothing else needed, no nebs etc. The little guy is very poorly today, agitated and hyperkinetic (due to Zinnat?) and confused when sleepy and in his sleep. I am struggling to control the fever, with Panado and Stopayne having no effect at all, and Ponstan bringing it down to 38.5. I gave him a Panamor supp last night, and that was effective - not sure how often I can give that though? When would hospitalization be indicated? I have contacted our Paed, but still waiting for his reply. I also highly value your opinion!

I am very concerned about your son and especially the fact that he is so agitated. This could be due to the pneumonia itself causing this high fever or to low oxygen levels in his blood [hypoxia] or to high CO2 levels in his blood [hypercapnia]. He should be admitted to hospital immediately. Please take him to an after hours emergency unit.Please let me know about his progress.
