perioral dermatitis

hi Dr. my 2 1/2 yr old son has this rash around his mouth. he had it over a year ago and i eventually took him to a dermatologist who prescribed him anti biotics and said it was the only thing that would get rid of it which it did. the rash looks the same now, he is cutting molars and drooling excessively. i know the steroid creams make it worse and mustnt be used but i am just wondering if you have any advice other than anti biotics which i would really like to avoid. i have tried epizone but its not going away. thanks

This kind of rash which is often called 'lick eczema' is very difficult to treat. It is important to be sure that your son does not have a constantly blocked nose and that he is not mouth breathing. If he is mouth breathing use a saline nose spray twice day to clear his nose. This rash is not an infection so antibiotic creams are not at all effective. Apply a very mild steroid ointment such as Mylocort ointment before bedtime and also apply a good brand of lip-ice to his lips.
