Baby''s twitch

Dear Professor

I have an eight month old little boy. He is very content, strong and healthy. About a month ago on a Sunday morning after having breakfast (8:45am), he was very tired and I put him down to rest. He was coughing quite a bit and my mother kept going on about it, so I decided to give him 1.5 ml Telfast to dry his nose up a bit. Now this was prescribed by the pediatrician on a previous occasion. I only used it the 4 days prescribed and then not again until that morning. Anyway, after I gave him the Telfast he was still unsettled and I gave him his milk (9:30am). He drank most of it and fell asleep. I covered him with one of his blankies and about a minute later I just checked on him and his whole body was shaking. Now, it wasn’ t big convulses that look like a fit, but it was def there. Then I just grabbed him and yelled and my parents came running and I didn’ t know what to do, but my mother just blew on his face and he opened his eyes and looked at us. He didn’ t choke or gasp for air. A couple of minutes later he had a burp and some milk came out, but not lots. I went to the GP immediately, who checked him out. He said he looked 100%. He didn’ t have a fever and his bloodsugar levels were fine, but said just to safe I should take him to the pediatrician. On the Tuesday I took him to the ped who then checked everything out and said he didn’ t find any reason to have to do a ECG unless it happens again. So, I obv still don’ t know what happened. My mother said his feet were a bit blue, so maybe he wasn’ t breathing, but I have to be honest in that I totally freaked out and can’ t really say.

Thankfully thus far, it has not happened again. But what I’ m worried about is that it seems that he has a twitch. But it happens so randomly and I that I can’ t really tell if it is voluntary or involuntary, because he is crawling now and very busy and he doesn’ t ever sit still, so I’ m not sure whether he is just flexing is muscles every now and then. Nothing else is wrong, he has a health appetite and he is still happy. Should I just relax about this, or should I have it checked out?

Thanks so much!

You have given an excellent history, but it remains difficult to be sure about what was the cause of your son's possible fit and now his twitchy episodes. The role of the Telfast in his first episode is not clear, but in any case this medicine should not be given to children under 2 years as it has not been tested for safety in this age group. It is necessary to be sure that your baby does not have some form of epilepsy. For this reason he should go back to the paediatrician who should definitely arrange for an EEG to be done this time.
