Hi Dr
Tayden (tubefed, 3yrs old)went for grommets and hearing test yest and all went well. After the op he had a husky voice which l thought was purely due to the tube being down his throat during the op.

Later in the afternoon, he started coughing and feeling unwell, quite normal after an op l thought. I put him to bed early and fed him through his mickey (Stopyane and Pediasure) at 8pm. He was quite restless, coughed quite a bit during the night, and his chest was rattling. This morning he woke up with a temp (38.4) and as he sat on our bed and ate some soft bread he started coughing, pulled a face as if he tasted something bad and then took out the bread which had streaks of purple Stopayne on it. How is that possible if l fed him the stopayne 10 hours previously?

This has happened another 4 times this morning, each time with purple stopayne appearing when he coughs. No major phlegm coming up, but I can hear it in his chest.

I am beginning to think that he has perhaps aspirated some of the stopayne / pediasure l gave him? Is that possible? I chatted to the paed and he has prescribed an antibiotic and meds to nebulise him. My concern is that he has started aspirating again. He was in hospital with Strep throat last week and faecal loading and managed to vomit up bread and apple juice in the hospital (he had Nissen anti reflux op in 2009) so l know that since last week he can vomit through the op.

Do you think the op has worn off and that he could be aspirating again? I am pretty nervous about this, since I feed him at night while he is sleeping.

What can I do about this and isn’ t it now dangerous to feed him while he is sleeping? He usually gets at least 500ml of fluids a night while sleeping so to cut that down is not an option.

This does sound like Tayden is refluxing again. His symptoms are very suggestive that this is his problem.Please do not give him Stopayne which contains codeine and will aggravate his constipation. Only give him Panado for fever.The only option is to prop him up as much as possible when you feed him and he should remain in that position for several hours after his feeds.Is he able to sleep propped up? You also need to discuss other options with his paediatrician such as giving him Motilium to try to prevent him refluxing.
