On reviewing your concern it is warranted that you return to your attending practitioner who conducted the procedure. Basically the tension of the eardrum grips the grommet around it's waist.

The cotton-reel shape stops the grommet from falling in or out. With the normal process of healing, when the eardrum heals over the grommet is pushed out. Sometimes when a grommet comes out, it may sit in the ear canal.

Here, some assistance may be required {by the appropriate health professional} in it's removal. An Ear Nose and Throat {ENT} specialist may be the best to assist/consult with in this regard, especially if the grommet has become dislodged precariously.
