From what you have written it is possible that your boyfriend has ADHD; however, the only person who can decide on a doctor's appointment is him. It seems to me that you have only his best interest at heart and would be there to support him. Having said this, walking up to him and saying "I think you have ADHD, here's the doctor's number" may be very upsetting to him, especially if his multi-tasking, although not always effective, does not bother him; this would need to be handled very sensitively.
