It does seem that you have tried everything, I think that the real culprits are the safety boots. The toe cap will cause excessive sweating which in turn creates a perfect environment for fungal spores to flourish. I suggest you treat the boots. Get some formaldahyde from the pharmacist, put a couple of drops on two cotton balls and place one in each shoe. Now put the boots in a black plastic garbage bag, tie it closed and leave it overnight. This will effectively kill all spores in the boot. Make sure that you always use fresh socks and rotate the boots. Good luck!
