There are some specialists known as Vascular Surgeons, who specialize in surgery on blood vessels, who might be able to advise. But presumably a neurologist would be the first port of call, who, if he felt the physical findings warranted it, could arrange for special X-rays to show the head and neck blood vessels, to see if there are any blockages, and if so, where. Not particularly pleasant procedures, though.
