Neck &  shoulder pain

I am a 50 year old male, who started feeling pain in my left shoulder about 10 days ago. It was of sudden onset &  gradually started affecting my neck &  upper back. I am right handed, but use my left arm a great deal when fishing, which is usually every 2 weeks. I also spend long hours at the computer, doing administrative work. My pain is worse at night &  I cannot lie down on my left arm, as this aggravates my pain. I also sometimes fell a clicking in my shoulder. I don' t experience any numbness or pins+needles. I have been to my GP, who said that I was suffering from stress, prescribed Voltaren &  recommended I attend physio. I have now attended 5 treatments &  my neck symptoms have improved, but my shoulder/arm pain is getting worse. She has diagnosed supraspinatus tendonitis/impingement &  has described my condition as an overuse injury, most likely brought on by fishing, that will take time to heal. However, I also feel excessive pain in my neck &  arm when she mobilizes my spinal joints (C4-C7, &  T3-T5) &  also in my arm when she mobilizes my AC joint. She has recommended that we continue physio as she says I still have severe muscle spasm &  many tension points in my neck &  upper back muscles. Should I continue treatment? Should she not be sending me for X-rays of the neck or shoulder? Why is my neck pain improving, but my shoulder pain getting worse? Please help.

Dear Sam
I think you have suffered enough to deserve to see an orthopedic specialist! Yes I agree, you may need more examinations - possibly a MRI of the shoulder rather than x/ray. You may need a cortisone injection in the shoulder.

Thank you for your response. My physio actually recommended I see an orthopaedic specialist at my next visit. She also spoke to me about the cortisone injection, but I thought I might need a better diagnosis (further investigation), rather than treating the symptoms with the injection. She has arranged an appointment with a specialist for next week. Thank you for your input.
