If your INR is stable (which may be problematic in 5 weeks), and as long as you are not breathless and your Oxygen Saturation at room air is normal, then it might be safe to fly.
You have to get the OK from your specialist. You may well have to have your INR tested while away. Generally one would say this is a bit short time period to do a flight longer than 4 hours.
