Continuous breakthrough bleeding

Since May this year, I' ve had excessively long periods whilst on Melodene. Had a regular period in July, and then spotted for a week from 19/08, then a full period until 5/09. During this time, my GP changed me to Nordette on 21/08. Since 19/08, I had a 23 day period, a 3 day break (while on nordette) then a 7 day brown discharge. Between August and September I skipped the placbo tabs.

Went to see a gynae who told me i have polycystic ovaries, and sent me for the following tests: prolactin, thyroid, glucose, insulin. I was told that my glucose insulin ratio indicated borderline insulin resistance. I then saw a dietician who put me on a low GI eating plan, which I followed successfully for 2 months, with regular periods. Redid the glucose and insulin tests after the 2 months, with positive results, i.e. my sugar levels have normalised. Right now, I again have my period a week before my placebos. What are the other possible causes? Please advise!
