The choice of medication is dependent on the nature of the problem. In patients who generally have an elevated estrogen environment such as polycystic ovarian syndrome and in whom there is no endometrial (lining of the womb) problem then Clomid is a suitable and cost effective choice. However in patients of more advanced age and who have a generally low body estrogen reserve may benefit more from the Femara. The main effect is not on the ovary as such but mainly on the endometrial.
