Dear Raffie
When patients lose very large amounts of weight, they often have a problem with stretched skin that will not respond to exercise to tighten it up. In such cases the patient is often referred to a cosmetic surgeon to have these folds of hanging skin removed. It would be best for any such surgical procedures to be carried out once the patient has lost all the weight he or she intends to lose.
