A GP should be able to arrange it, or of course a psychiatrist if you are already seeing one. The real issue is the diagnosis and assessment of your condition and of how best to manage it -if hospital admission is considered a good idea, it will be arranged.
The sort of problems you describe could all respond really well to treatment, both with medicines and with poper skilled counselling, like CBT, and might not need hospital admission. But this will be decided by your doctor, bearing in mind the silly rule your medical aid apparently insists on. You can also explore what facilities might be available to you through your nearest medical school dept of psychiatry or nearest major state / privincial hospital
