Treatment Endometrial intraepithelial neoplasia - Hysterectomy with adnexectomy or not ?

Good morning. Can you say me your opinion on the case report. Should a woman - 42 years old with endometrial intraepithelial neoplasia undergo a hysterectomy together with adnexectomy? Patient history: normal BMI, metrorrhagia because of EIN, which was diagnosed by curettage,2 children,anxiety disorders- takes antidepressants Please contact me: k.nouzova@centrum.cz

I would think that the next step would be a hysteroscopy to check the whole endometrial cavity as  an area of cancer could have been missed at curettage. If this shows only EIN then it would also depend on whether there is atypia or not. As the patient is symptomatic a hysterectomy would probably be indicated. I would suggest that you discuss with a  gynae oncologist about whether adnexectomy is necessary as well.
