The risks with a 4th caesarean are that the uterus can rupture if she goes into labour. Therefore another caesarean must be done before labour. The surgery is likely to be more difficult because of the previous surgery and there is more risk of damage to bladder or bowel. Terminating the pregnancy is a bit drastic though, unless there are some underlying medical reasons why she had the previous caesareans.
