When using contraceptives - what are the causes for the 1% pregnancy rate?

Oral contraceptives are 99% effective when taken correctly, and absorption is optimal but there may be times when perhaps they are not taken at the correct time or perhaps there is decreased absorption from the gastrointestinal track because of diarrhoea or other medications taken which decreases absortion of the pill.
